Sample Acceptance Conditions | İstinye University

 

1. For each analysis sample, the relevant analysis request form must be fully completed and signed. Additionally, the service agreement must be read. 

2. Samples to be analyzed must be delivered or sent in appropriate containers, secured against leakage or spillage, and packaged in a way that they are not affected by the container. The packaging must include a name or code identifying the sample. Samples not submitted in appropriate containers will not be analyzed, and any responsibility arising from this will belong to the applicant. 

3. Samples requiring cold chain during transportation must be delivered to the laboratory without breaking the cold chain. 

4. Analysis samples must not be harmful to human health or the environment. Toxic, carcinogenic, explosive, or radioactive samples that may cause poisoning through contact or inhalation are not accepted. In suspicious cases, the laboratory authority must be consulted. 

5. Special storage conditions of the sample (cold chain, light-proof, airtight packaging) must be stated in the Analysis Request Form. The laboratory cannot be held responsible for structural deterioration in samples where such conditions are not specified. 

6. The analysis period is a maximum of 15 working days from the date the samples are accepted. The duration may be extended depending on workload. In case of delays due to device malfunction, workload, etc., notification will be provided. 

ISUBITAM TEST SERVICE AGREEMENT 

1. This agreement is a service contract between Istinye University Scientific and Technological Research and Application Center Laboratory and the applicant requesting the analysis. The laboratory is referred to as ISUBITAM.

2. Sampling in accordance with test conditions is the responsibility of the applicant.

3. Until acceptance by ISUBITAM, transportation, packaging, and storage of the sample are the responsibility of the applicant. ISUBITAM cannot be held responsible for negative results arising from these factors.

4. Samples that do not meet acceptance criteria will not be accepted by ISUBITAM.

5. The applicant must code and list the samples starting from 01. The coding must be written on the samples in a permanent manner.

6. In order for applications to be accepted, the relevant Test Request Form must be fully completed and signed by the project coordinator/authorized person.

7. By completing and signing the Test Request Form, the applicant declares that they have stated any harmful effects of the sample on human and environmental health and accepts responsibility for any issues if not declared.

8. Declared test durations are estimated. ISUBITAM is not responsible for delays due to unforeseen circumstances. In case of deviation, the customer will be informed.

9. For scheduled tests, the customer agrees to be present at the specified laboratory at the appointment time and to notify at least one day in advance if unable to attend; otherwise, the test fee will be charged.

10. Samples requested to be returned will be delivered with the test report. If not collected within 15 days, they will be disposed of. Unless otherwise stated, samples that can be stored will be kept for three months and then disposed of.

11. All shipping costs belong to the customer.

12. Test reports will not be issued without proof of payment.

13. The customer agrees that test results apply only to the tested sample, will not be used for commercial purposes, and will not imply ISUBITAM approval.

14. One test report is issued per analysis. Additional reports or formats are subject to extra fees.

15. If repeated tests due to objections yield the same results, the full service fee will be charged again.

16. ISUBITAM does not track project details (SANTEZ, KOSGEB, BAP, EU, TÜBİTAK, etc.). Legal responsibility for non-compliance belongs to the project owner and customer.
